The CPA Committee mandates that all photography/videography projects provide and complete this release form for their subjects and artists.
To properly use this form, please do the following:
-Download this form to your desktop
-Fill out the entire form (except the signature field) and save a copy
-Print a physical copy of the form and have the subject and photographer/videographer physically sign the form
-Give the subject a copy of the signed form and keep a copy for yourself
